Expense Form Instructions This is to report expenses that are not reimbursed but donated. These donations will appear on your giving statement.

In-Kind Form This is when you need to report a donation that you do not want to be reimbursed.

Minor Media Consent Form Children must be opted in to appear in our social media postings. Parishioners are welcome to opt out if they desire. This form is used in both cases.
